Single Incision Single Port Robotic Right Middle Lobectomy

BACKGROUND: Single port robotic technology (SP) was approved for thoracic surgery in the United States in June 2024. There is limited experience in subcostal anatomic lung resections and even more limited experience in completing these procedures without the use of an accessory port for stapler placement and without the use of a robotic stapler.
METHODS: A single incision SP subcostal right middle lobectomy was performed.
RESULTS: We demonstrate the ability to perform a SP robotic right middle lobectomy through a single 4 cm subcostal incision.
CONCLUSIONS: The SP robotic platform allows for safe completion of a right middle lobectomy through a single 4 cm incision without the need of an accessory port and without the need of a robotic stapler. By performing this procedure through a single subcostal incision, we avoid entry in the intercostal space. We suspect that this will lead to improved patient postoperative pain, although further studies will be needed to investigate this. The description of this approach may aid others in adapting this technique.
