Bileaflet Mitral Valve Prolapse: A Difficult Problem With An Elegant Solution

BACKGROUND: Bileaflet mitral valve prolapse is widely regarded as one of the most complex valvulopathies to repair. The nuances of this complex valve repair can be accomplished via minimally invasive techniques.
METHODS: This video details our robotic approach to repairing anterior and posterior leaflet prolapse. A right anterior mini thoracotomy incision in the 4th intercostal space with three additional working ports are used to approach the mitral valve through interatrial groove. P2 triangular resection and plication of the A3 segment onto A2 were used. A 35 Duran flexible posterior annuloplasty band was used.
RESULTS: Post bypass there was no residual mitral regurgitation, no SAM and a mean gradient of 3 mmHg.
CONCLUSIONS: Even in bileaflet mitral valve prolapse, a successful and elegant repair can be accomplished using minimally invasive cardiac surgical techniques.
