Robot-assisted Coronary Artery Bypass Grafting In A Single Academic Center: Preliminary Results

BACKGROUND: The robot-assisted (RA) techniques for surgical coronary revascularization have been developed to get a less invasive alternative to conventional coronary artery bypass grafting (CABG), to improve postoperative recovery, patient satisfaction, and early discharge. This is particularly useful in selected patients that could benefit from hybrid revascularization. This study aimed to compare the feasibility of RA-CABG with conventional CABG.
METHODS: The study accrued 32 patients who underwent RA-CABG and 18 patients who underwent conventional CABG for single and multi-vessel disease between January 2019 and July 2022. The primary outcome measured was the number of patients who were discharged to their homes within five days after the operation. Secondary outcomes included the total length of hospital stay, reoperations within 48 hours, transfusions and 30-day mortality.
RESULTS: The results showed that 31% of the RA group was discharged to their homes within five days, compared to 22% of the conventional CABG group (P<0.01). The median post-operative hospital stay was also shorter for the RA group (4 days) compared to the conventional CABG group (7.5 days) (P<0.01). The 30-day mortality rate was 3% in the RA-CABG group versus 5% in the conventional CABG group. Other secondary outcomes did not differ significantly between the groups.
CONCLUSIONS: RA-CABG was feasible and associated with a higher rate of early discharge to home. Shorter hospital stays without a difference in 30-day mortality. This approach may reduce healthcare resources and provide a consistent framework for hybrid coronary revascularization.
