Redo Mics Opcab In A 68 Years Old Male 20 Years After Undergoing Conventional Cabg

BACKGROUND: Redo CABG is still associated with high mortality and morbidity.Option other than Redo Sternotomy like Minimally Invasive Cardiac Surgery thru a Left Anterior Small Thoracotomy and Off Pump Coronary Artery Bypass can be a safe alternative.I'm sharing our first experience of this modality in our country.
METHODS: A case of 68 years old, male presenting with chest heaviness, palpitations and progressive dyspnea, known diabetic, hypertensive and previous smoker, post-CABG thru Midline Sternotomy with Saphenous Vein Graft to RCA(1999).Latest Coronary Angiogram done a month before CABG. showed CAD-2 Vessel Disease involving the LAD and RCA. A CT-Angiogram was done prior to the operation as anatomic roadmap.He underwent Redo MICS OPCAB with Left Internal Mammary graft to distal LAD and Saphenous Vein Graft to distal RCA (March 12,2020)
RESULTS: Postoperative course was essentially unremarkable, had cardiac rehabilitation and discharged on the 9th hospital day improved
CONCLUSIONS:Redo MICS OPCAB Surgery is feasible and acceptable in patients undergoing Repeat Surgical Coronary Revascularization
