TRANSAXILLARY APPROACH TO AORTIC VALVE

BACKGROUND: Transaxillary approach to aortic valve is superior to other methods of minimal invasive surgery in selected patients
METHODS: This noval surgical technique was initially started as an experimental approach after having adequate experience and learning curve in minimal invasive mitral valve surgeries through mini anterior thoracotomy .The main decision is based on radiological assessment of the anatomy of aortic valve annulus and its angle to the longitudinal axis of ascending aorta and the distance from annulus to the third and fourth intercostal space .
RESULTS: The results are superior with regards to early post operative recovery, lesser need of blood transfusions,lesser pain, early mobilisation and superior cosmetic results .Even technically in the operative field the exposure to right coronary cusp and its subvalvar muscular portion is well visualised so that the ease of decalcification and inadvertent chances of conduction tissue injury is less . Sizing of the valve , cardio pulmonary bypass and aortic cross clamp time are on par with conventional aortic valve replacement by standard midline sternotomy CONCLUSIONS: Thus transaxillary minimal invasive approach to aortic valve is superior in selective patients with favourable anatomy . It may allow many newer attempts to address aortic and arch pathologies in future .
