Hybrid Approach To A Distal Arch Aneurysm

BACKGROUND:A 63 year old gentleman presented with a saccular aneurysm of the arch of the aorta. He had vocal cord dysfunction from pressure on the recurrent laryngeal nerve which led to the diagnosis. We planned to place a covered stent across the mouth of the aneurysm. The arch was diseased needing a zone zero landing. He had significant iliac disease too.
METHODS: Surgical debranching was done and a graft was sewn onto the ascending aorta to facilitate the stenting
RESULTS: A covered stent was placed across the mouth of the aneurysm from the ascending aorta to the descending aorta. Patient recovered well and is fine on follow up
CONCLUSIONS: Hybrid procedure decreases the morbidity in tackling suitable arch aneurysms
