Off-pump Endartrectomy

Background: The aim of this study is to review our center experience in off-pump coronary artery endarterectomy.Methods: Between January 2014 and April 2020 a total of 15 off-pump coronary endarterectomies was performed. The mean age was 66 ± 10 years and mean LVEF was 40%± 5 % and all of the patients were in New York Heart Association (NYHA) III to IV. Patients had an average follow-up of 36± 12 months Intra-operative details included Number of targets were 3± 1.Results: Intra-operatively left anterior descending (LAD) endarterectomy was performed on all cases. the number of targets was 3± 1 with no conversion to on-pump procedure and all planned grafts were performed achieving complete revascularization. No in-hospital mortality, mean ICU stay 4 ± 1 day (d), and length of stay (LOS) 7± 2 d. We had no Strokes and 1 postoperative MI. 4 years follow-up showed functional class NYHA I in 80% (n=12)II in 13%(n=2) III to IV in 6.6%(n=1), freedom from reintervention was 93%(n=14).Conclusions: Our experience shows that Off-pump endarterectomy is safe and feasible associated with good midterm outcomes and freedom from reintervention.
