Minimally invasive aortic valve replacement through a right mini-thoracotomy is our preferred approach for isolated aortic valve replacement. We illustrate a replacement of the ascending aorta above the sinotubular junction and of the aortic valve, without circulatory arrest, through a small 5cm right anterior mini-thoracotomy in the second intercostal space.
