Nine Years Results Of Minimally Invasive Transthoracic Device Closure Of Doubly Committed Subarterial Vsd

BACKGROUD In recent years, minimally invasive Off-pump transthoracic device closure (MIOTDC) has been introduced as an alternative treatment option for selected patients with doubly committed subarterial ventricular septal defects (dcsVSDs). We retrospectively reviewed 68 dcsVSDs cases received this treatment in our center and summarized the clinical experience and mid-term results.METHODS Between January 2008 and June 2017, 68 patients with dcsVSDs, with a mean age of 13.5±8.3 months and body weight of 9.2±6.7 kg, who met the inclusion criteria were enrolled in this study and received MIOTDC under general anesthesia and transesophageal echocardiography guidance. All the successful patients were strictly followed up according to a standard protocol, by 2 doctors who were specially appointed.RESULTS 63 cases were successfully closed(92.6%)and the other 5 patients (2 with aortic regurgitation, 2 with residual shunt due to device malposition and 1 with right ventricular outflow tract obstruction) were converted to surgical closure. No severe complications (severe arrhythmia, ventricular outflow tract obstruction, or more than moderate valvular regurgitation) were observed during follow-up period. CONLUSIONS In select patients, MIOTDC of dcsVSDs appears to be safe and effective, with good midterm outcomes.
