Robotic Thymomectomy Infiltrating The Left Innominate Vein

OBJECTIVE: Robotic approach has several technical advantages over manual videothoracoscopy system, it offers an high definition three dimensional view and robotic arms are more comfortable to use, allow more precise, flexible and intuitive movements
METHODS: We present the technique of a robotic thymomectomy en bloc with tangential resection of left innominate vein, resection of left upper lobe and dissection of the phrenic nerve vessels in a 75 yy patient with 5 cm clinical T4 B1-B2 thymoma.
RESULTS: The postoperative course was uneventful and the patient was discharged in postoperative day 2.
CONCLUSIONS: Robotic technique allows radical resection of mediastinal masses even when an infiltration of innominate veins and other surrounding organs is discovered intraoperatively and an extended surgery is required.
