First experiences with late stage activation in patients with ischemic MR

OBJECTIVE: Longterm success after mitral valve repair in patients with ischemic MR is a challenging task due to the problem of recurrent mitral insufficiency. With the development of adjustable mitral annuloplasty rings the ability to treat these patients has been improved.
METHODS: Participating in the DYANA II study 12 patients have been already included at our institution. MV repair was performed using the enCore SQ annuloplasty ring. This device consists of a memory shape alloy core. A permanently attached lead is placed subcutaneously and allows for postoperative adjustments in a minor surgery setting.
RESULTS: A 73 year old lady with coronary artery disease and ischemic MR underwent three vessel revascularisation and mitral repair using the encore sq device. Three months postoperatively progression to severe MR was noted. Late stage activation was successfully performed and resulted in almost complete resolution of her mitral valve regurgitation.
CONCLUSIONS: A technology that adjusts the shape of an annuloplasty device, without the need for a second open surgery, and via minimally invasive procedure, can address this significant unmet clinical need.
